TERMS & CONDITIONS FOR REGISTRATION AT MAVIS TUTORIAL CENTRE

I, the parent/guardian (“Parent”) of the student (“Student”), agree to the following terms and conditions (“General Terms”):

TERMS AND CONDITIONS:

1. Registration fee, course fees and deposit will be collected together upon registration.

2. Registration fee and course fees are non-refundable.

3. Students will only be entitled to the course materials starting from the first paid lesson until the last paid lesson.

4. Fees are to be paid within the first week of the month and are collected monthly regardless of lesson(s) attended, with the exception of Clause 6. Fees are not transferable to other month(s) regardless of circumstances.

5. For the year end, Secondary 1 to 3 students will have to pay 1.5 months’ fees (to be collected in Sept) for September and October. Primary 6 and Secondary 4 Normal students will have to pay 2 months’ fee (to be collected in Aug) for August and September. Primary 1 to 5 and Secondary 4 to JC 1 students will have to pay 2 months’ fee (to be collected in Sept) for September and October.

6. For new registrations commencing in September, a two-month fee will be collected in advance.

7. Fees will not be refunded should a student miss lesson(s) unless it is due to the centre’s default. Never miss a lesson again. PRO: Recorded Lessons – Access recorded lessons: anytime, anywhere, Online Lessons – Attend online lessons: same lesson, same day & time.

8. Request to transfer class due to genuine reason(s) may be approved once proper permission is obtained from the Support Department of the centre (subject to seat availability). Approved change of class will commence from the following month.

9. Students who wish to terminate classes must log in to our withdrawal portal at cs.mavistutorial.com to submit the request at least ONE calendar month in advance (before or on the 1st of the said month) but with the exception of Clause 10.
Once the termination date crosses beyond the 1st of the said month, it will not be considered as a full month’s notice.

10. Clause 9 will not be applicable for the last 2 months of the academic year for all levels.

11. Deposit will be forfeited if one calendar month’s notice is not given.

12. Deposit is non-transferable. It can only be used to offset the last month’s fee and is only valid for 3 months, after which it will be forfeited. For refund of deposit, parents are required to verify at the counter personally.

13. Should the deposit be forfeited, it must be recollected for students with an active course enrolment.

14. Students who stop lesson for a month or more and rejoin the centre will be charged the usual registration fee, course fees and deposit, thus treated as new students.

15. No lessons are conducted on public holidays, Chinese New Year’s Eve, Teachers’ Day and end of October. Fees will not be pro-rated for the stated closures.

16. The centre reserves the right to transfer, combine, terminate a class or change the class teacher at its own discretion.

17. Persistently disruptive behaviour during lessons or non-compliance with the Centre’s regulations, will lead to expulsion of the student from the centre.

18. Personal data collected will be used mainly for identification and registration purposes, updating of upcoming events/workshops/
intensives/seminars, handling of administrative and operational aspects within or between our centres, marketing, branding and communication purposes and requesting for feedback or participation in surveys. Personal data collected may be disclosed to our staff for effective discharging of work or teaching obligations.

19. Student/Parent is to furnish any changes to personal particulars to the centre accordingly. Failure to do so may hinder you from receiving relevant updates and will not be the responsibility of the centre.
